The Rogue vs. The Moose (2011) Pilot/Crew Decision Making - Series #1 Video length - approx. 20 minutes. WARNING - This video contains actual footage from a fatal accident and may not be suitable for all audiences. This video is the first in the Pilot/Crew Decision Making (PDM/CDM) Series produced by System Safety Services and challenges the viewer to recognize the links that form to cause the accident. The video then demonstrates the links and finally the safety nets that can help prevent a repeat of the accident. This video is designed to be a training video for aviation pilots, particularly General Aviation pilots. This video is based on an actual accident. It uses actual video from the pilot's camera to illustrate the links that lead to the fatal accident. An experienced pilot (2,800 hours) makes a fatal error in judgment that results in five fatalities. The video shows the many links that lead to the final error.
